Information you voluntarily provide via Google Forms is used to register for lessons, or be included in Music for Veterans Project material. This information will be stored for an indefinite period of time. If you wish to have information you submitted on these forms be permanently removed, please use the contact page and I can delete your Google Form responses. Information submitted through the forms will only be used so that you can be contacted regarding lessons or MVP and the information you provide is that which you personally decide to share with me. The content you provide on any of the MVP forms will be used to create a printable learning module for a student composer assigned to compose your commemorative piece.
